Notre survie d\u00e9pend d&rsquo;elle et des substances qu&rsquo;elle contient. Nous d\u00e9pendons \u00e9galement du plasma, la partie liquide du sang qui, en plus des sels, des nutriments, etc., transporte les d\u00e9chets que nous devons \u00e9liminer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Et c&rsquo;est l\u00e0 que nous arrivons aux reins, organes vitaux qui nettoient le sang en filtrant<em> environ 180 litres par jour<\/em>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Leur importance est r\u00e9sum\u00e9e par le dicton populaire selon lequel quelque chose de tr\u00e8s cher (ou qui demande beaucoup d&rsquo;efforts) \u00ab\u00a0co\u00fbte un rein\u00a0\u00bb. La t\u00e2che principale des reins est l&rsquo;\u00e9limination des d\u00e9chets et la r\u00e9gulation des fluides. Il contient un filtre \u00e0 deux parties, l&rsquo;une pour le sang lui-m\u00eame et l&rsquo;autre pour le liquide de dialyse : le dialysat.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le sang et le dialysat entrent en contact l&rsquo;un avec l&rsquo;autre, s\u00e9par\u00e9s par une membrane. Au d\u00e9but du traitement, une petite intervention chirurgicale est pratiqu\u00e9e sur le patient pour pouvoir r\u00e9aliser la dialyse.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le sang circule en sens inverse du dialysat, qui le nettoie par diffusion. Il y a g\u00e9n\u00e9ralement 3 sessions de 4 heures par semaine.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les \u00e9l\u00e9ments les plus gros et les plus importants du sang (cellules sanguines, prot\u00e9ines, etc.) restent dans le sang qui arrive dans le dialyseur.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les d\u00e9chets tels que l&rsquo;ur\u00e9e, la cr\u00e9atinine (et les ions tels que le Na et le K) diffusent \u00e0 travers la membrane dans le dialysat et sont \u00e9limin\u00e9s. Le liquide de dialyse peut \u00eatre modifi\u00e9 en fonction des besoins de chaque patient.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>L&rsquo;incident de Caruaru<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Caruaru est une ville mini\u00e8re de l&rsquo;\u00c9tat de Pernambuco au Br\u00e9sil.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Au d\u00e9but de l&rsquo;ann\u00e9e 1996, apr\u00e8s une longue s\u00e9cheresse, la ville a subi une coupure d\u2019eau courante. Source: <a data-type=\"URL\" data-id=\"https:\/\/commons.wikimedia.org\/wiki\/File:Barragem_Tabocas.jpg\" href=\"https:\/\/commons.wikimedia.org\/wiki\/File:Barragem_Tabocas.jpg\" target=\"_blank\" rel=\"noreferrer noopener\">wikimedia commons<\/a>.<\/figcaption><\/figure><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">L&rsquo;eau utilis\u00e9e pour la dialyse provenait du r\u00e9seau municipal d&rsquo;approvisionnement, pr\u00e9lev\u00e9e dans le r\u00e9servoir de Tabocas, \u00e0 une quarantaine de kilom\u00e8tres.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Apr\u00e8s la purification pour l&rsquo;approvisionnement public, les cliniques r\u00e9alisant la dialyse proc\u00e9daient \u00e0 une purification suppl\u00e9mentaire en la faisant passer par des colonnes de sable, de carbone et de r\u00e9sines \u00e9changeuses d&rsquo;ions, avant de la faire passer par un filtre \u00e0 micropores. Les colonnes \u00e9taient r\u00e9g\u00e9n\u00e9r\u00e9es tous les 3 jours.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><strong>Mais la s\u00e9cheresse a modifi\u00e9 le cours des \u00e9v\u00e8nements<\/strong><\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Lorsque l&rsquo;approvisionnement public est tomb\u00e9 en panne entre le 13 et le 17 f\u00e9vrier, l&rsquo;une des cliniques a re\u00e7u de l&rsquo;eau de Tabocas par camion-citerne. Le seul pr\u00e9traitement a consist\u00e9 \u00e0 ajouter de fortes concentrations de chlore, puis la clinique a produit le dialysat selon la proc\u00e9dure habituelle.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La clinique de l&rsquo;Instituto de Doen\u00e7as Renais (IDR) comptait 126 patients dans son programme d&rsquo;h\u00e9modialyse. D\u00e8s que les patients ont re\u00e7u une dialyse avec cette eau, ils ont d\u00e9velopp\u00e9 des sympt\u00f4mes neurologiques (d\u00e9ficience visuelle, naus\u00e9es, vomissements, etc.), et des dommages au foie ont \u00e9t\u00e9 relev\u00e9s chez 89% des patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Le premier d\u00e9c\u00e8s est survenu le 20 f\u00e9vrier. Le 6 mars, il y a eu 10 victimes. Ensuite, l&rsquo;unit\u00e9 de dialyse a \u00e9t\u00e9 ferm\u00e9e et les patients ont \u00e9t\u00e9 transf\u00e9r\u00e9s \u00e0 Recife. Mais le mal \u00e9tait d\u00e9j\u00e0 fait. Le 4 ao\u00fbt, 55 personnes \u00e9taient mortes, le bilan final \u00e9tant de 60 victimes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les d\u00e9c\u00e8s ont \u00e9t\u00e9 caus\u00e9s par des l\u00e9sions h\u00e9patiques ou indirectement par des complications telles que la septic\u00e9mie, les h\u00e9morragies gastro-intestinales et les probl\u00e8mes cardiovasculaires.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><strong>Que s\u2019est-il pass\u00e9\u00a0? (2016).<\/figcaption><\/figure><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Dans les premi\u00e8res analyses de s\u00e9rum sanguin, on a d\u2019abord recherch\u00e9 des contaminants tels que des m\u00e9taux lourds, des pesticides, des chloramines&#8230; mais rien d\u2019anormal n\u2019a \u00e9t\u00e9 d\u00e9tect\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mais lorsque la pr\u00e9sence de toxines cyanobact\u00e9riennes a \u00e9t\u00e9 sugg\u00e9r\u00e9e, les r\u00e9sultats ont montr\u00e9 la pr\u00e9sence de microcystines (-YR, -LR et -AR). <em>La microcystine-LR \u00e9tant la plus toxique.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les microcystines sont des heptapeptides (constitu\u00e9es de 7 acides amin\u00e9s) cycliques. Plus de 200 variants sont actuellement connues.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Elles ont des effets h\u00e9patotoxiques puissants et inhibent les prot\u00e9ines phosphatases chez les mammif\u00e8res, avec des cons\u00e9quences graves (par exemple, dommages \u00e0 l&rsquo;ADN et n\u00e9crose cellulaire).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Elles favorisent \u00e9galement les tumeurs du foie chez les animaux de laboratoire. <em>Un d\u00e9sastre.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Des \u00e9tudes pathologiques sur des \u00e9chantillons de foie de patients atteints ont montr\u00e9 des alt\u00e9rations identiques \u00e0 celles observ\u00e9es chez les animaux domestiques et sauvages intoxiqu\u00e9s par les microcystines (c&rsquo;est-\u00e0-dire des plaques de cellules h\u00e9patiques rompues et d&rsquo;autres d\u00e9formations).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les cyanobact\u00e9ries dominantes dans le r\u00e9servoir de Tabocas sont tropicales. L&rsquo;analyse des communaut\u00e9s phytoplanctoniques des ann\u00e9es pr\u00e9c\u00e9dentes a montr\u00e9 que les cyanobact\u00e9ries (<em>Microcystis, Anabaena<\/em> et <em>Cylindrospermopsis<\/em>) \u00e9taient dominantes depuis 1990. Mais une premi\u00e8re enqu\u00eate men\u00e9e apr\u00e8s l&#8217;empoisonnement a r\u00e9v\u00e9l\u00e9 des populations phytoplanctoniques diff\u00e9rentes entre mars et mai 1996, comprenant <em>Aphanothece, Aphanizomenon, Chroococcidiopsis<\/em> et <strong><em>Microcystis<\/em><\/strong>, entre autres genres.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Des \u00e9chantillons pr\u00e9lev\u00e9s fin mars 1996 ont \u00e9t\u00e9 analys\u00e9s quelques ann\u00e9es plus tard et ont montr\u00e9s que les cyanobact\u00e9ries repr\u00e9sentaient 99% de la communaut\u00e9 phytoplanctonique, avec plus de 20 millions de cellules\/litre (principalement <em>Aphanizomenon manguinii<\/em> et deux esp\u00e8ces <em>d&rsquo;Oscillatoria)<\/em>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les principaux producteurs de microcystine sont bien s\u00fbr <em>Microcystis<\/em>, mais aussi <em>Planktothrix, Oscillatoria, Anabaena<\/em> et <em>Aphanizomenon<\/em>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les microcystines sont les seules cyanotoxines pour lesquelles l&rsquo;OMS a publi\u00e9 des directives sanitaires, avec des limites de s\u00e9curit\u00e9 fix\u00e9es dans l&rsquo;eau potable \u00e0 1 \u03bcg\/L\/jour de microcystine-LR (et un niveau d&rsquo;alerte moyen de 20 \u03bcg\/L).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Jusqu&rsquo;\u00e0 10 \u03bcg\/L de microcystines ont \u00e9t\u00e9 d\u00e9tect\u00e9s dans le s\u00e9rum des patients intoxiqu\u00e9s et on estime que l&rsquo;eau de source en contenait jusqu&rsquo;\u00e0 19,5 \u03bcg\/L.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Des restes intacts et des fragments de microalgues et de cyanobact\u00e9ries ainsi que d&rsquo;autres cyanotoxines (cylindrospermopsines) ont \u00e9t\u00e9 trouv\u00e9s dans les colonnes de filtration d&rsquo;eau de la clinique.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les traitements d&rsquo;h\u00e9modialyse n\u00e9cessitent environ 120 litres d&rsquo;eau par personne. Vous pouvez imaginer que la concentration de cyanobact\u00e9ries et de cyanotoxines \u00e0 Tabocas \u00e9tait plus que suffisante pour provoquer une intoxication aigu\u00eb chez les malheureux patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L&rsquo;incident de Caruaru a soulev\u00e9 la n\u00e9cessit\u00e9 d&rsquo;inclure les cyanotoxines dans la surveillance de l&rsquo;eau et d&rsquo;am\u00e9liorer les techniques de traitement avant dialyse.
